The Great Dalmuti turns your table into a rigid little kingdom every single round. Play cards low to high, whoever runs out first becomes the Great Dalmuti and everyone else slides into their new social rank, right down to the miserable Greater and Lesser Peons who must hand over their best cards and worst seat. Then you play again, and the deck itself is stacked so the low cards are common and the high card is desperately scarce, meaning a lucky opener can coast while everyone else scrambles. It is card-shedding chaos with a mean little class system bolted on, and reversals of fortune are half the fun.
